Is there an excessive amount debited from your OV-chipkaart? You couldn’t check in or out because of a malfunction in the OV-chipkaart system during the trip? Or did you forget to check out? Then there are several ways to reclaim your overpaid money.
Four euros were debited from your balance when you checked in. HTM will repay the amount you paid in excess. To do this you will need a claims form and a summary of the last ten transactions. You can download the form here, request it by telephone from the HTM Consumer Service Line 0900-HTMINFO/0900-4864636 €0,10pm) or collect it in person from HTM Customer Services. Use one form per person per claim.
• Manual Claims Form: download and fill in this manual claims form (Dutch). Don’t forget to enclose a printout of the incorrect transaction. This can be printed on ov-chipkaart.nl or at a pick-up device. Find a pick-up device nearby.
Complete the claims form and send it with the required enclosure to: A stamp is not required.

• Digital Claims Form: the digital claims form (Dutch) can be filled in en sent to HTM directly online. If you are entitled to a refund, HTM will credit the relevant amount to your OV-chipkaart. you must collect this amount as an order from an pick up device. To apply for a refund by digital refund form, you need to fill in the internal card number of your OV-chipkaart. This number is available from the public transport customer service points.
• Claims Form ‘Mijn HTM’: do you have a “My HTM” account? Apply for refund by using the claims form ‘Mijn HTM’ (Dutch). This version allows you to store personal information such as name and address in your ‘My HTM’ account. This can be useful for your refund requests in the future.
• HTM Customer service: you can always visit HTM Customer service points for more information or assistance to reclaim your overpaid money. They also print out a transaction summary for you. Don't forget to bring your OV-chipkaart with you. The money will be paid to your OV-chipkaart directly.
Your request for a refund must be received by HTM within three months of the relevant missed check-out. If you have paid too much, € 4.00 minus the cost of the trip and minus the fixed fee of € 0.86 will be paid to your OV-chipkaart. Don't forget to collect this refund as an order from a pick up device before this amount can be used for your public transport journey.
